Cysts and lipomas are common benign skin lumps that can sometimes require surgical removal. This document provides an overview of these lesions, surgical options, and post-operative care.

 

1. What Are These Lesions?

a) Epidermal Cysts (Sebaceous Cysts)

  • Form due to a blocked hair follicle or sebaceous gland.
  • Appear as firm, round lumps beneath the skin, sometimes with a central punctum.
  • May become inflamed, infected, or rupture, requiring treatment.

b) Pilar Cysts (Trichilemmal Cysts)

  • Develop from hair follicles, most commonly on the scalp.
  • Feel smooth, firm, and mobile under the skin.
  • Less likely to rupture than epidermal cysts but can grow large.

c) Lipomas

  • Benign tumours made up of fatty tissue.
  • Soft, slow-growing lumps that move easily under the skin.
  • Usually painless, though large lipomas may cause discomfort or cosmetic concerns.

    2. Surgical Options

    a) Cyst Removal

    • Involves making an incision and carefully excising the cyst and its sac to prevent recurrence.
    • Local anaesthetic is used for a painless procedure.
    • If infected, drainage and antibiotics may be required before removal.

    b) Lipoma Removal

    • A small incision is made, and the lipoma is gently dissected and removed.
    • Suitable for symptomatic, rapidly growing, or cosmetically concerning lipomas.
    • Larger lipomas may require deeper dissection.

    What to Expect on the Day of Surgery

    • The procedure is performed under local anaesthetic in a sterile setting.
    • The area will be cleaned and numbed before an incision is made.
    • The cyst or lipoma is carefully removed, and the wound is closed with sutures.
    • The procedure typically takes 20-45 minutes.
    • A dressing will be applied, and aftercare instructions will be provided.

    4. How to Look After the Wound Post-Surgery

    • Keep the wound clean and dry for the first 24-48 hours.
    • Follow instructions on dressing changes to prevent infection.
    • Take mild pain relief (e.g., paracetamol) if needed.
    • Avoid strenuous activity that may strain the wound.
    • Stitches are usually removed within 7-14 days, depending on the location.
    • Watch for signs of infection, such as redness, swelling, pus, or increasing pain.

    Most cyst and lipoma removals heal well with minimal scarring. If you have concerns about your wound or notice any complications, contact your healthcare provider.
